Siyan Clinical Corporation is inviting you to participate in a survey about the emotional impact of COVID-19 on you and your household members. With your help, Siyan can learn more about how the COVID-19 pandemic has impacted patients and help identify areas of needed support and services.
With funding from the Substance Abuse Mental Health Administration (SAMHSA) FOA #FG-20-007, Siyan’s evaluation partner LPC Consulting Associates, Inc. will gather and analyze survey results.
